Legal name change help!?
In some sections it says petitioner (present name) but in one section it just says "Petition of (name)" Now would i put my birth name or the name im changing it to??

You are the petitioner. That means you are submitting the petition. It's YOUR petition. Both are the same.

Same name in both spots (assuming there is some other spot for you to put your 'new' name).

Birth name. That's your name until the petition is reviewed and granted.
