Yes they do when the manic phase goes to the extreme and a person's judgement is severely impaired or they become highly delusional.
If the Bipolar patient stays well regulated on their daily medications they can usually avoid these extremes and the need for antipsychotic medications.

Yes, the medications/SSRIs help with the symptoms of mental illness, you won't necessarily feel better though.
