    The slope is 3 and the perpendicular slope is -1/3

    I concur with you.

    (0 - (-3)) / (5 - 4) = 3/1 = 3 slope (given line); perpendicular line would have a negative reciprocal slope, which here is -1/3.  D) confirmed.
