Yes, even if you worked just a few days, they are supposed to send it to you or give it to you.

Yes. The company is required by federal tax law to give a W2 to every employee they paid wages to during the year.
Generally they will allow you to pick it up in person or access it online if their systems are set up for that. But if they don't hear from you by the end of January they will mail it to the address they have on your file. If you've moved then make sure you either give them your new address, or you have mail forwarding set up with the post office (or get it online or in person).

Yes. Your former employer will mail your W-2 by January 31 to the address you told them to mail it to.
If you've moved, you were supposed to notify them of your new address by the end of December.
