Strange how Americans use this structure. In British English we just ask, "Have you ever had a tiny tattoo?"

Since "ever" means "at any time in your life up to this moment", we don't need to say "before".

No, I don't have any tattoos or want any.

Yes but it has nothing to do with boosting confidence. I had that before getting the tattoo. Tattoos rarely have any thing to do with boosting confidence. To me tattoos are more about getting closure, or remembering some one or some thing special.
