    If they feel there's something they consider fraud then yes, otherwise such as non payment they would usually give advance notice.

    Yes, absolutely; the commonest reasons are if you run out of credit on a pre-pay account, or go over any usage limits.

    If you are sticking to the rules, then the next most likely things are dirty SIM contacts, faulty SIM or a fault with the phone itself.

    They can't even illegally deactivate the SIM card. They CAN deactivate your service and I bet not responding to their notice is the reason.
