In many situations, it is good to put a comma after it, but there are probably many exceptions as well.

Some punctuation guidelines (this is a punctuation question, not a grammar question) say that the comma after a short introductory phrase is option. However, "all of a sudden" is not that short, and in any case, the comma always makes the meaning clearer. Think about how you would say the sentence- normally, your intonation would drop at the end of "sudden". Punctuation is meant to show intonation changes in spoken language.

Yes, sentences that begin with the phrase "All of the sudden" should have a comma or an ellipsis after the phrase. In fact, the only time "all of the sudden" shouldn't be followed by a comma or an ellipsis would be if it is the end of the sentence, in which case it would be followed by a period, question mark, exclamation point, or period-ellipsis. Example(s):
All of the sudden, there was a knock at the door.
All of the sudden... there was nothing; it was all gone.
Everyone had been waiting for many hours or even days, and then, all of the sudden, the doors opened, and the sale was over in minutes.
I saw her wearing the other team's uniform; she had changed schools all of the sudden.
Why had the dog become ill all of the sudden?
So now, he's a well-respected attorney all of the sudden!
The car just died all of the sudden....

You don't need a comma after that phrase. You could put the phrase in another position, like "There was a knock on the door all of a sudden". If you put it first, you still don't need a comma.
