how can utilitarian thinking be seen in history's worst events?

Ask yourself whether the majority of the population perceived it as an improvement on their previous existence, then it must be "better". This is clearly the case under Stalin – serfdom, world wars, revolution and civil wars had made life deeply unpleasant for the majority. Under Stalin, although millions were killed, the majority were living in better conditions and receiving better (any) education.
So although it clearly was not a great way to exist it was an improvement, for the majority, on what came before, therefore it must be better.
