Do I have the right to give my consent at the dentist to have them share information with a family member over the phone or in person ?

    They might ask you for a notarized letter granting that permission, but probably.

      You really are the Queen of bad answers.  AGAIN, where do you get your information, specifically, this information - a notarized letter?  Ever hear of HIPAA?

    Of course, you can give your dentist permission to share your information with anyone you want.

    I believe the dentist would want written consent.

    in the US, if you are adult, of course..you would have to sign a form, but the info can be shared in person or over the phone
