    If the government is paying for the provider, the government decides. If the patient is paying for the provider, the patient decides.

    Yes, she can. A person can refuse to accept a carer who makes them uncomfortable.

    Yes. I knew one that would only hire white middle-aged women. An agency taking their money is going to try to make them happy.

    You can ask, but none may be available.

    Of course women are much more likely to be available than men. A guy asking for male only caregivers is probably out of luck, not because of gender ethics but because of who is likely to have such a job.

    I would point out that such a request is basically catering to an old lady's sexism, but you've got a pretty good "that ship has sailed" counter-argument.
