Do you have to sleep on your back in the hospital (I’ve never seen anyone in a hospital setting sleep any other way)?

You don't have to sleep on your back.

Because the beds are adjustable it is easier to get comfortable on your back when awake.

I adjusted mine to flat before rolling on my side to sleep when I was in hospital.

You can sleep in a hospital bed in any position you want to sleep in (on your back, on your side, on your stomach - whatever position you sleep in at home). You would only be forced to sleep on your back if you've had some sort of medical procedure that makes it imperative that you sleep on your back.

Nah, you can sleep anywhere you like, even in the reception as well
