Is it possible for someone to have a 7 month old and a 4 month old?

I suppose it's possible, if the woman gets pregnant again almost immediately after her 7-month-old was born.

Yes, in a medical situation where one child is delivered early, and the other goes closer to full term.

Years ago (I can't find the story now), before the McCaughey septuplets were born, a woman was pregnant with seven children and one was born very early and didn't survive while the other six were delivered months later and did survive.

Actually - it is possible but extremely rare.

The woman would need to be pregnant for twins (or more). And there would need to be a medical situation in which one baby is delivered premature but the other baby is not delivered at that time. The birth of the second baby can be delayed usually by two to four weeks. In one rare case, the twins were born 87 days apart and both survived. At 87 days, the age gap would be about three months meaning that a woman would have a 7 month old and a 4 month old at the same time.
