It means underweight. However, I have heard it used many times as a complement, meaning slim. If you can't tell what was meant from context, you could ask. Or, if you like, you could assume it was meant as a compliment, whether or not it actually was.

The 'desirable' level is 'slim'. 'Skinny' is below that: where you're on the verge of looking unhealthy, and your head looks too big for your head, and your eyes look too big for your head, and people wince when they see your pathetic spindly bones.

Usually "skinny" and "thin" have slightly negative implications, whereas "slim" is usually positive. "Underweight" is almost "medical" and is definitely negative. Note that, in the 1960s the model known as "Twiggy" was definitely underweight, but had a good career.

The best thing to do is to google with search key "catwalk models". Look at the women and you got the perfect explanation of skinny. Most of them only eat cereals to make a living....

Depends who's saying it. It's a relative term.

Skinny implies that you look unhealthily underweight.

Slim means that you look to be at a good weight for your height and build.

weight is different for everyone. Some with long bodies and short legs can carry more weight that someone that is all legs and a short body. But they usually gage for women the measurements equal height. Say your 5' 4" or 64" tall then your waist should be 26" around and your hips 38" They do not judge bust in that equations. So around you equal what height you are.

Skinny is below slim. Skinny means you look underweight, whereas slim does not. That's how I have always categorised it.
