It's going to depend on the hospital and program, but normally a resident would start working in early summer -- late June, say.

If you're writing fiction, you can make up a date in late June or early July.

Most interns start working on July 1st, but I suppose it could vary be region/hospital.

''A year in residency begins between late June and early July depending on the individual program, and ends one calendar year later.'' https://en.wikipedia.org/wiki/Residency_(medicine)

Of course there is an exact date. They are not told to "start sometime next month".

What that date is (which is not your question) will vary with hospital, city, country, etc.

A good writer who hopes to actually get published does not write about things they know nothing about. If you cannot place your main character in a setting that is familiar to you then you're wasting your time. Asking on here does not constitute adequate research.

    In which country?

But before you try writing a book, please learn to spell.
