Provident Mutual Life Assurance Association demutualised in January 1996, and was taken over immediately by General Accident Linked Life Assurance Ltd.
Following the merger with Commercial Union in October 1998, the company changed its name to CGU Linked Life Assurance Ltd; subsequently becoming Norwich Union Linked Life Assurance Ltd in October 2000 after the merger with Norwich Union.
Its registered head office is in York. The company is owned by Norwich Union Life Holdings Limited, which is ultimately owned by Aviva plc which is incorporated in England.
CGNU was renamed Aviva plc on 1July 2002.
General Accident's Life offices were originally in York, prior to all the renaming.
So you want to get in touch with Norwich Union Linked Life Assurance Ltd , whose head office is in York. There is still a fund separately called the "Provident Mutual Fund" set up in 2005, which is a sub fund of the Norwich Union Life and Pensions fund.
