If said car is on their property, yes it is. Drive off of their parking lot.

Yes they can, smoking is not a right it's a priviledge, especially when it comes to someone's property. Illinois just started enforcing the law in Jan, 2008, no smoking in any 'public' building, or within 15 feet of any entrance to a public building. State law. That's where Barrack Obama is from. But, anyway, they are passing more laws like this all the time in other places.

Yes, it is legal. It is on private property and they can dictate policy for their establishment, which can extend to any parking facilites on premesis.

Many counties and some states even have ordinaces against smoking in public now and makes it illegal to light up pretty much anywhere expect you home or the personal dwelling of another that allows you to smoke in their home.

Every hospital I know of, has the same rule, including being in your vehicle. Their property, their rules. Our schools have the same thing.

If it is private property then the answer is YES. This might be challenged in the Supreme Courts someday.....but for NOW the answer is STILL yes.

Yes it is. A hospital is a place to get well. Not kill yourself slowly. Why should they allow you to smoke anywhere on the grounds. That's like them promoting smoking.

Being in your own car is your business, as long as it's not on someone else's property.
