This causes mild to moderate central canal stenosis. This may affect the descending bilateral L5 nerve roots. There is mild bilateral neural foraminal narrowing due to broad-based disc osteophyte complex and degenerative facet arthrosis. L5-S1: No focal disc protrusion or extrusion is seen. There is no central canal or neural foraminal narrowing. ** IMPRESSION **: 1. L4-L5 large central disc protrusion with mild to moderate central canal stenosis.     Last summer, I took a bad butt fall while zip lining. Initially the pain was horrible but didn't last long. Four months later, I began to experience sciatica pain. Going from one doctor to another is NOT for me so I tried acupuncture just once. The acupuncturist inserted the needles in various areas near spine and buttock. Since she was too far from my home, I tried an acupuncturist nearby. He said it was sacroiliac and inserted the needles in areas of my hand. The needles were thicker and he actually "screwed" them deep into my hand. This was 2 days ago and I really cannot tell if that made any difference. He also told me that the stretches I had been doing for sciatica were the worse thing I could have done since there is no muscle in the sacrum area. The pain most often settles in my outer ankle!     Yes, I know I have already posted a few questions with regards to this matter but I am still very confused. I am being sued in California by a credit card company for unpaid credit debt. I do not deny the debt but there are extenuating circumstances. I am required to "answer" the summons. I spoke to an attorney who advised that there is really no legal form although it must be in "proper legal form." He also stated that when I mail the copy of my "answer" to the attorney, I must have someone other than myself mail it. He said I cannot fill out the proof of Service. I disagree. I do know that a plaintiff must have someone else serve papers but I am almost certain that the defendant can respond themselves. Please, please.. any help at all? I have navigated the Internet for hours looking for a sample format with which to enter my "answer" to the summons. I find nothing. Thank you so very much.

I make very little money and have no assets nor access to resources from which to borrow the money. What is typical or standard judicial procedure? If my paycheck is garnished, I will not be able to pay basic living expenses. I work for a school district. Last year, our contract was ratified which included a pay decrease and furlough days.     I am in California. In August of 2009, the school district for which I work, ratified a new contract which drastically reduced my income. I wrote letters to all of my creditors. I told them I would no longer be in a position to pay my monthly minimum payments but would send what I could. I did this and for several months the companies cashed the checks. Last night I was served a summons informing me that one of the banks is suing me. I must respond within 30 days. The summons states that I must use a legal form but I have no idea what form(s) to use. I've looked on the court info website but it is very difficult to navigate and I found nothing. Please... any solid and serious advice would be greatly appreciated.
