Does anyone know the name of the form which you can get at a Garda Station which shows proof of your identity?

It's for people who don't have a passport or driving licence.

Just go in and ask for the form that shows proof of your identity.

You can get the National Age Card at your Garda station, it's designed to be a proof of age card not an identity card

You can get your National Age Card at the Gardaí station but its not for people who dont have a passport or driving license its because some places only take garda identification when you are buying products only legal for people over the age of eighteen such as shops that sell beer example aldi only take the age card.
If you have no passport from any country you will be deported due to legal reasons and may be banned from the Republic Of Ireland.
The form you use to apply is called the "National Age Card Application Form".
