How Do I Post A Letter From The USA To The UK?

I'm writing a letter to one of my friends that lives in the UK and I just need to know what to put on the envelope.
All I need to know is how to write the address.
For the USA I know we write it as:
(Persons first and last name) John Smith
(House #, Street name) 555 Pleasant Street
(city, state, zip code) Boston, MA 55555
(country)United States.
How is it written for the UK?

An address.... is an address!
What exactly do you think the reason is that we "ALL" have them?
All have the persons first and last name, a street number and street name, a city or town name a country name and an area code.
That's "WHY" we ALL have adresses...so mail "ALL" done the SAME WAY... in the "SAME" form...."ALL OVER THE WORLD"

....gets there!

THINK!!!!!!

by the way...put the proper postage on it for an overseas letter.
...& no you don't need an England stamp. A stamp on a letter is from the country the letter is FROM...not the country the letter is going too.
....is this all too much for you?
