When was the New York smoking ban initiated?

Hello my lovely people. Could you please tell me:

a) When was the smoking ban law initiated in New York City,
b)What did it initially include? (ie workplaces etc)
c)And when was it amended to forbid smoking in any public space (If I'm not mistaken)

In short, I'm looking for all the information I can get regarding the NY smoking ban law and its history :)

I think it was in 2003 when you couldn't smoke anymore in a bar.
